Casper Women’s Care Closing Notice
Casper Women’s Care will be closing permanently on October 17, 2025.
If you need a copy of your medical records, please complete the Medical Records Request Form and return it by:
- Drop-off at our office
- Fax - (307) 237-0607
- Bringing in a USB drive for our team to download your records
Our address is 940 E 3rd Street Suite 202 Casper, WY 82601. Our hours are Mond-Thurs: 8:30am to 4:30pm, and Fri-Sunday: Closed.
If you have already selected a new provider, we can forward your records directly to them once we receive your completed request form.
Please note: The Phreesia Payment Portal below will remain available on this site for now for patients to pay outstanding balances. Please reach out to us at (307) 237-5510 for any questions.
