New Patient Information

If you're a new patient with us, here are some quick tips to know before your first appointment:

Photo ID and Insurance Info

All new patients must provide a photo id in order to be seen - we also need current insurance information, or full payment may be needed if unable to verify benefits.

Guests

We only allow 1 guest over the age of 18 to accompany the patient to our clinic, in order to give you the patient the best uninterrupted and safest care possible. Thank you!

Patient Forms

If the patient is over the age of 18 they must be the contact number and fill out the Phreesia documents. Minors between the age of 13 and 17 must sign a consent to release any information even to a guardian (Wyoming State Law). While we recommend you fill these forms out on Phreesia, you can also download the forms directly from our site here:

Phreesia Pre-Check In

You will receive a message to your phone or email on file 4 days prior to the scheduled appointment. Once the link is opened, you will need to enter your birth date, and answer multiple choice question about your address. Once these patient identifiers have been answered, please do patient “paperwork” and precheck in via your phone or computer prior to appointment.


Any new or established patient: in order to receive a secure email, you must register for the patient portal, provide email address to us via texting, call or when in office and request we send you an invite email to register for a portal account. You can also make payments through Phreesia as well!

Solution Reach

Solution Reach is a software that allows us to communicate with you via text message. New patient’s will get Text reminders of their appointments, as well as annual reminders to schedule their well woman exam in the future. You can CONSENT to texting with office via this texting software about medical questions, bills, appointments, etc. The office will answer during normal business hours.

EZ Access Patient Portal

Our patient portal is a secure online website that gives you your own access to your personal health information whenever you made need it. The online tool is designed to help you easily view your health information and clinical results.


Any new or established patient: in order to receive a secure email, you must register for the patient portal, provide email address to us via texting, call or when in office and request we send you an invite email to register for a portal account.
